Nominations are now open for the Victorian Disability Sector Awards 2012.

The awards celebrate and acknowledge those who demonstrate excellence, passion, vision and commitment to assisting people with a disability achieve their life goals.

Nominations

Members of the public can nominate an individual, team or organisation making a significant contribution to improving the lives of people with a disability in the following eight award categories:

1. Support Worker Award (Individual)
2. Support Worker Award (Team)
3. Client Services Worker Award
4. Volunteer Award
5. Leadership Award
6. Emerging Leader Award
7. Business Supporting Disability Award
8. Lifetime Achievement Award

Nomination Process

Read the Nomination Guidelines 2012, before completing the nomination form.

Complete and save the Nomination Form to your computer with your last name, first name (e.g. Smith_John.doc)

Your completed nomination form should be attached and emailed or posted to:
